Job Summary
We are looking for an experienced Childrens Mental Health Practitioner to join our Central Basildon PCN for 12 months fixed term. As part of the NHS long term plan, local areas were asked to realign community mental health services with primary care networks, creating new and integrated models of primary and community mental health care.
PCN's Aim To: Have a key co-ordinating role at the heart of the GP practices and are a fully integrated member of the multi-disciplinary team with regards to response and management pathways; Provide a combined consultation, advice, triage and liaison function in accordance with agreed protocols; Support children and young people with mild to moderate mental health problems through evidence-based interventions.
PCNs are a new service designed to support children and young people. The Mental Health Senior Practitioner will be responsible for the assessment and caseload management within the GP PCN. They will triage cases, complete a full assessment and manage their workstream. The post holder will work closely with the operation manager and Clinical & Strategic Lead, to ensure effective delivery of the PCN initiative in Essex.
Contract and Location Fixed term for 12 months. Location: Central Basildon PCN, Fryerns Medical Centre, Basildon, SS14 3SS. Some travel around the county is expected for meetings or training events.
Main Duties
This CYP MH Practitioner post supports the development and delivery of the PCN service in Essex. The post holder will deliver evidence-based low intensity interventions in GP services, assist liaison between professionals, and work with GPs to support children and young people with mild to moderate mental health difficulties and their families.
The post holder will have experience delivering individual interventions and group support for children and young people with mild to moderate mental health needs, managing a clinical caseload and meeting service targets. They will work with the service manager and clinical lead to facilitate wider service transformation within the local child mental health system.
The Post Holder is expected to collaborate with the Service Manager and Clinical Lead and participate in available training opportunities. Core clinical responsibilities (examples):
* Work as part of a multidisciplinary team to assess and manage patients using expert knowledge
* Assist in the assessment of new patients and develop/complete individual care plans
* Provide specialist clinical support and advice to young people with mental health difficulties
* Support engagement with services and treatment, including onward referrals or signposting
* Educate and involve family members in treatment as necessary
* Plan day-to-day personal workloads
* Follow departmental policies and guidelines
* Triasge and receive referrals within the PCN
* Communicate effectively with patients using appropriate methods
Qualifications and Experience
Essential
* Experience: At least 2 years delivering evidence-based psychological interventions in mental health with children and young people
* Experience assessing children and young people with a range of presentations (low mood, anxiety, OCD, phobias, early psychosis, developmental conditions, etc.)
* Experience working with more complex presentations and neurodivergence
Desirable
* Experience working in primary care settings
* Experience delivering clinical interventions in different cultural contexts
Work arrangements
Essential: Ability to travel efficiently throughout the area.
