Overview
Manager with a passion for enhancing the quality of life of the people we support is at the heart of our community service teams. You will lead, support and empower a dynamic team in Flintshire, helping people to gain independence in daily living.
Responsibilities
* Develop and implement person‑centred support plans.
* Conduct care needs assessments.
* Induct new colleagues and lead supervisions/appraisals.
* Complete medication competencies and audits.
* Attend multi‑disciplinary meetings and prepare reports.
* Assist the Branch Manager in day‑to‑day service management.
* Participate in the on‑call rota (subject to service needs).
* Visit services regularly and spend time with people we support.
Qualifications & Experience
* Previous experience in a care management role, ideally within CIW.
* Level 3 qualification in Health & Social Care.
* Experience working with individuals with autism, acquired brain injury, complex mental health needs and distressing behaviours.
* Strong values and passion for improving lives of others.
