Responsibilities
* To proactively manage and liaise with clinical and administrative teams regarding all suspected and diagnosed cancer patients to ensure each stage of their pathway is fully planned and booked to meet Cancer Wait Times targets.
* To work as a key part of the Multidisciplinary Team (MDT) to ensure the cancer patient and carer experience is central at every stage of the journey.
* Ensure MDT meetings are run efficiently, are well‑structured, cover all cancer patients, and that the whole patient journey is recorded in the Patient Pathway Management (PPM) database.
* Participate across a range of functions including supporting Audit, the Cancer Reform agenda, National Service Frameworks and Clinical Governance.
* Work with external organisations to ensure effective communication across service providers for the smooth transfer of cancer patients.
* Track patients and actively manage the clinical pathway to ensure appointments, investigations, MDT discussions and treatments are conducted on time according to their scheduled pathway.
* Actively monitor cancer waiting times for individual patients, identify risks, and take corrective action to keep national and local targets from being breached.
* Liaise with clinical teams to expedite interventions that may cause delays or lie outside expected timeframes.
* Escalate to line manager or General Managers where there is a risk of breaches in line with performance for cancer waiting times.
* Ensure accurate and concise information is recorded on the PPM database.
* Liaise with respective coordinators for other providers when patients are referred for further investigations or treatments to ensure all necessary information is transferred.
* Organise MDT meetings, including arranging room bookings, confirming equipment availability, and reporting any failures to the appropriate person.
