We have an exciting opportunity for a fully qualified and experienced social work practitioner to join Cumberland Council, working with adults who have a breadth of needs, which are likely to be complex in nature.

About The Role
The post is primarily to assess needs and in doing so, gather information which would evidence a primary health need and support the submission of a Checklist and subsequent Decision Support Tool, under the National framework for NHS continuing healthcare and NHS-funded nursing care.
Who We Are Looking For
The Ideal candidates will have a robust understanding of the principles of The Care Act 2014 and will be skilled in undertaking assessments and be able to effectively manage a mixed complexity case load.

This role requires a DBS check. The level of check for this role is - Enhanced DBS Check with an Adults and Children’s Barred List check.
Please note this role involves working in regulated activity with Adults. It is an offence for people who are barred from working in regulated activity to apply for roles that require them to work unsupervised with adults.
