Thepostholderwillundertakethefollowingin Acute Medicine:
Usetheirexpertknowledgeandcomplexdecision-makingskillsinunpredictable situations and making a differential diagnosis.
1. Completeafullsystemphysicalexamination.
2. Takeasystematicpatienthistory.
3. Screenpatientsforearlysignsofdiseaseandriskfactors.
4. Carryoutwhereappropriatearangeofinvasiveandnon-invasivediagnosticand therapeutic procedures to agreed levels of competence.
5. Orderarangeofdiagnostic’stestsandprocedures.
6. Makediagnosticdecisionsbasedonpathologyorimaging results.
7. Initiateprescribedtreatmentsandmedication.
8. Deviseanindividualcareplanthatincludesallaspectsofmultidisciplinary care.
9. Refertootherhealthprofessionalsasappropriate.
10. Dischargepatientsfromhospital/serviceorarrangefortheirtransferofcaretoanother health care setting.
11. Responsible for the leadership, of all enhanced and advanced practice staff, both traineesandqualifiedwithinAcute Medicine,aswellassupportingeducationalsupervisors and engaging with Higher Education Institutes (HEI’s) for the delivery of academic modules.
12. Lead on traininganddevelopmentoftheACPworkforce withinUHD Acute Medicine department
13. MaintainappraisalsandmandatorytrainingofstaffwithinUHDAcute Medicine ACP teamviadelegated teams.
14. Responsibleformonitoringofriskandgovernance withinUHDAcute Medicine ACP teamand implementation of any actions
15. ResponsibleforlocalcomplaintsandLERN withinAcute Medicine related to the Advancing Practice team.
16. Promoting research, quality improvement and audit within the ACP workforce to support service development.

Qualifications
Essential criteria
29. Masters in Advanced clinical Practice or PGDip as a minimum must include: History taking and physical assessment, pharmacology, Clinical decision making and diagnostics, Supplementary or independent prescribing, ionising radiation (medical exposure) Regulation trained, Evidence of continuing professional development, budget management, recognised teaching or mentoring qualification, ALS Provider
Desirable criteria
30. Recoginition as Advanced Practitioner by the National centre of Advanced practice, recognised leadership Qualification

31. A minimum of 5 years experience in relevant speciality, Experience of working in MDTs with in acute health care setting, extensive, demonstrable application of advanced clinical skills and experience in relevant specialty
