Provides coordinated care to patients of all age groups. Plans, implements, and evaluates patient care plans to restore or maintain patient well-being. Participates in the coordination of care. Qualifications Qualifications: Speech-Language Pathologist, licensed per the state requirements 2 years of Home Health preferred Good communication skills, assessment skills, and strong clinical documentation skills. Ability to read, write, and speak the English language Reliable transportation. Valid and current auto liability insurance Essential functions: Assess and diagnose patients with speech, language, hearing, oral motor, swallowing, and cognitive disorders. Develop and implement personalized care plans to address patient-specific communication and swallowing issues. Provide therapeutic interventions aimed at rehabilitating patients' speech, language, cognitive, and swallowing functions. Monitor and evaluate patient progress, adjusting treatment plans as needed to optimize outcomes. Collaborate with other healthcare professionals to coordinate care and ensure continuity of treatment across disciplines. Educate patients and families on therapeutic techniques, goals, and at-home exercises to promote recovery and independence. Document patient evaluations, progress, and care plans accurately and timely to meet clinical and legal standards. Communicate effectively with patients, families, and the healthcare team to ensure a comprehensive understanding of patient needs and progress. Travel to patients’ homes and provide services in a home health setting, ensuring reliable transportation and adherence to safety regulations.
