Integrated Care Clinician (Paramedic)
The post holder will undertake telephone triage assessments of patients efficiently, advising the most appropriate intervention to meet patient needs using best clinical practice, Trust policy, protocols, and guidelines.
Work autonomously, assessing and advising callers within agreed guidelines.
It is a new direction in your career with challenges in urgent and emergency care across Wales.
Main duties of the job
Work within a multidisciplinary team including GPs, pharmacists, and mental health practitioners to provide phone and online triage and assessment. Assist callers with minor ailments, injuries, and complex urgent conditions, making clinical decisions remotely.
Use your clinical assessment skills to advise patients and highlight ability to make autonomous decisions while being supported by senior clinicians and practice coaches.
Welsh language ability is desirable.

Qualifications and/or Knowledge & Experience
* NMC Registered Nurse (adult or child), or HCPC Registered Paramedic with post‑registration experience in front‑line services or telephone triage.
* Evidence of regular training and continuous professional development as required by the regulating body.

* Flexible approach to work – expect rotating shift system including nights and weekends (unsocial hours).
