We are looking for a Receptionist to work 12 hrs per week between 8:00 am and 7:00 pm. This exciting opportunity is for an experienced, enthusiastic, motivated and hard‑working individual to join our busy practice and provide an efficient, friendly service for patients and visitors, while delivering effective administrative support to our doctors and clinical team.
Main duties
* Communicate kindly, professionally and caringly with a wide range of people.

* Apply IT skills, with familiarity with GP practice systems desirable.
* Work with GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where appropriate, refer people back to other health professionals within the PCN.
* Raise awareness of shared decision‑making and decision support tools and support shared decision‑making conversations.
* Process appointment, home visit and telephone requests.
* Book, amend and cancel patient appointments.
* Process repeat prescriptions in accordance with practice guidelines.
* Initial contact with and respond to requests from patients, other team members and associated healthcare agencies and providers.
* File and retrieve paperwork; take messages and pass on information.
* Keep the reception area tidy and free from obstructions and clutter.
* Complete annual mandatory training.
Key responsibilities
* Utilise population health data to proactively identify patients.
* Support patients in preparing for shared decision‑making conversations.
* Holistically bring together identified care and support needs within a single personalised care and support plan (PCSP).
* Help patients manage their needs through answering queries, making and managing appointments, and ensuring that good quality written or verbal information is available to aid choice of care.
* Support patients to take up training, employment and appropriate benefits.
* Assist with access to self‑management education and support, personal health budgets and coordination of care across services.
* Assist patients in accessing personal health budgets, where appropriate.
* Provide coordination and navigation for people and their carers across health and care services.
* Support the coordination and delivery of MDTs within the PCN.
