Job Responsibilities
Participate as part of a team managing a new and dynamic service supporting individuals with complex needs, including mental health, physical health, and substance misuse issues.
Assess and manage risks for clients and the service, ensuring safety and well-being at all times.
Deliver high-quality care, support, and housing management in accordance with organisational policies and procedures.
Work collaboratively with partner agencies to provide holistic, wrap-around support tailored to individual needs.
Develop and implement client-centred support and risk management plans, aiming for suitable move-on options within six weeks.
Engage with clients to build positive relationships, motivate progress, and encourage participation in their recovery journey.
Manage challenging situations effectively, ensuring safety for clients and staff, and report safeguarding concerns promptly.
Maintain accurate and timely case records, update IT systems, and produce reports as required.
Contribute to a positive, inclusive working environment that values diversity and promotes equality.
Participate in rota shifts, including evenings, weekends, and bank holidays, to ensure continuous support coverage.
Support clients with housing benefit claims and other administrative tasks related to tenancy management.
Required Skills & Qualifications
Understanding of the needs of homeless people and experience working in a face-to-face support environment.
Experience working with individuals with complex needs, including mental health, physical health, and substance misuse issues.
Knowledge of trauma-informed care and move-on options for clients.
Excellent communication skills, with the ability to connect, motivate, and adapt approach to diverse clients and stakeholders.
Strong decision-making skills, with the ability to assess risks and respond effectively in challenging situations.
IT literacy, including proficiency with Microsoft Office packages and database management.
Ability to work independently and as part of a team, demonstrating self-motivation and a proactive attitude.
Relevant qualifications or certifications in health, social care, or related fields are desirable.
