Salary: £26,535 - £30,602 depending on skills and experience
Job type: Full time, 37.5 hours per week. Part time position will be considered. This can be discussed at interview stage. Working days Monday - Friday between 8am-8pm.
Location: The successful candidate will be based at Shipston Medical Centre. The role will involve remote working, but when necessary, and to meet business needs, the successful applicant must be willing to support all PCN practices. Due to the geography and nature of the role, a clean UK driving licence and full use of a car is essential.

Main duties of the job
DSV PCN are seeking a full time Social Prescribing Link Worker for the Shipston-on-Stour practice area. Part time would be considered for the right candidate.
This rural community has benefited from excellent social prescribing, and we would like to build on this success.
Based at the Shipston Medical Centre premises and other local sites, you will engage with people seeking support with many aspects of their life. There will be group and one-to-one sessions, sometimes in a clinical setting and sometimes in the patients home, requiring good planning and organisational skills, as well as an up-to-date knowledge of local, regional and national resources.
Excellent communication skills, a mature approach, and confidence in working with a wide network of people are needed for this role. You will receive training, support, and regular supervision, and be part of an active and supportive practice team.
This is a full time post, with flexibility to meet service needs. Start date May 2026.

Job responsibilities
Social Prescribing is a freeservice to help people improve patients general health and wellbeing in aholistic way. Through using non-medical sources of community support, SocialPrescribing offers the chance to find out about activities that can improve health and wellbeing, including advice and information services, communitygroups, leisure activities, lunch clubs, self-help groups, specialist interestgroups, sporting activities, and lots more.
As a link worker you will be working closely with the Health and Wellbeing Team, taking referrals from the surgeries, placed Based Teams, and other third-party roles.
A link worker will develop relationships in the health and social care sector, working closely with our Health and Wellbeing Coach, Care Coordinator and Clinical Pharmacists, and supporting individuals to access voluntary and community opportunities.
You will be responsible for providing non-clinical personalised support to individuals and their families, in order to empower them to take control of their own health and wellbeing, give them time and focus on what matters to them, in order to holistically improve an individual's health and wellbeing.

Experience
* Experience of supporting people, their families and carers in a related role (including unpaid work).
* Experience of data collection and providing monitoring information.
* Demonstrable commitment to professional and personal development.
* Able to listen, empathise with people and provide person-centred support in a non-judgemental way.
* Able to get along with people from all backgrounds, respecting lifestyles and diversity.
* Able to work to policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
* Personal accountability, emotional resilience and works well under pressure.
* NVQ Level 3, Advanced level or equivalent qualifications or working towards.
* Experience of supporting people with their mental health, either in a paid, unpaid or informal capacity.
* Experience of partnership/collaborative working and of building relationships across a variety of organisations.
