An opportunity has arisen for an experienced paediatric nurse to join our Paediatric Immunology & Infectious Diseases CNS team.
We are seeking a children's nurse who demonstrates good clinical skills, has significant paediatric experience, good leadership qualities and a high level of organisational and communication skills to join our Paediatric Immunology & Infectious Diseases multidisciplinary team (MDT).
We are looking for an organised, innovative individual to work as part of a dynamic team in providing excellent care, education and training to families and staff virtually, by telephone, face to face and email.
You will act as a resource providing expert advice for healthcare professionals and must be competent to deliver health promotion and education to parents, their families and others.

Main duties of the job
To act as a coordinator and facilitator of joined up care for patients under the children living with HIV service, the prevention of HIV transmission from mother to child service, the 22q service, the immunology service, and the congenital infection service.
A direct point of contact for patients who have condition-specific queries, medication requests or require acute review and/or assistance/advice with symptom management.
To carry out and subsequently report on diagnostic investigations. Review results, discuss as appropriate with MDT and feedback to child and family. Arranging necessary follow up as required
You will be an expert communicator and educator, able to manage complex situations and support children and families through clinic attendance and out of hospital support. Liaising with other centres and all our community partners are key to safety and progression of our patients' care. You will have the opportunity to develop and learn but experience in this area is advantageous.
Our CNS role is an integral part of the MDT working together to deliver this specialist service, promoting treatment pathways that empower & support children and families to manage their condition and help to minimise hospital admissions.

Detailed job description and main responsibilities
Clinical Responsibilities
1. work autonomously, managing a caseload of patients within the specialty whilst working as part of the multidisciplinary team, delivering individualised and personalised direct patient care
2. Be responsible for assessing and recognising emergencies in the speciality. To interpret information and take appropriate action and to lead others to do the same.
3. Act as an advocate for patients within the department, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines.
4. Ensure that patients’ receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines.
5. Recognise and avoid situations that may be detrimental to the health and wellbeing of the individuals.
6. Actively engage service users through the facilitation of user groups and open days.
7. Co-ordinate the provision of specialist services to patients, both within hospital and/or in the community setting, effectively liaising with multidisciplinary colleagues
8. Develop and run nurse led clinics where the post holder will co-ordinate the treatment in conjunction with medical staff, and provide treatment which may include minor surgical procedures for patients.
9. Direct, support, encourage and advise clinical teams, by the provision of specialist knowledge and advice in the care of patients within the area.
10. Participate in the promotion of patient centred care, and establish and maintain a supportive relationship with the patient and their family during the period of assessment, treatment and after care
11. Support and enable patients and carers to make informed decisions relating to their treatment and management.
12. Promote high quality clinical care, and assist in the development of the specialist service, through the use of evidence based practice and clinical standards
13. Provide professional and clinical leadership and expertise in the speciality and to act as a resource for education and clinical expertise in the hospital and community settings.
14. Demonstrate appropriate expert practice and highly specialist advice in the assessment, planning, implementation and evaluation of patient care. This includes history taking, physical examination and the requesting of agreed diagnostic procedures/clinical investigations.
15. Promote and deliver high standards of care at all times, which utilises research-based evidence, and is audited and improved accordingly.
16. Participate in the promotion of the specialist service through dissemination of specialist knowledge, both locally and nationally.
17. Act as a role model, demonstrating high standards of care and providing clinical leadership in the specialist areas.
18. Establish, develop and maintain effective working relationship between clinicians, practitioners and managers within the specialty, across the Trust and in the community
19. Actively participate in local, national and international networking to maintain and develop expert level of knowledge in the specialty.
20. Provide a seamless, high quality service from referral through to assessment, diagnosis, treatment and review.
21. Develop and evaluate assessment tools that will ensure an appropriate level of nursing intervention so that patients who present with the most complex needs receive the most appropriate specialist care.
22. Work in conjunction with a clinical consultant to lead a patient focuses service.
23. Act as patient advocate when appropriate, respecting patient confidentiality and privacy with respect for diverse cultural backgrounds and requirements.
