Nurse Practitioner Job Description
Job Summary
We are offering an exciting opportunity for dedicated and compassionate Advanced Nurse Practitioner (ANP) to join Warrington Innovation Network (WIN PCN) and support the delivery of our Same Day Access Service.
This is a patient-facing, autonomous clinical role working across a set weekly pattern at fixed locations within our member GP practices and PCN in Warrington. The ideal candidate will possess a strong background in patient care and be adept at providing high-quality medical services. The postholder will work alongside a skilled multi-disciplinary team and will be supported by both clinical and non-clinical colleagues within the PCN and practices.

Key Responsibilities
* Provide consultations for a range of acute and chronic conditions, including assessment, diagnosis, treatment, and prescribing
* Act as an Independent Prescriber, working within professional and national guidelines
* Deliver evidence-based care and manage complex patient presentations
* Order and interpret pathology and diagnostic tests as appropriate
* Provide health promotion and preventative care, identifying and supporting patients at risk of developing long-term conditions
* Manage and prioritise clinical caseloads, ensuring safe, timely, and effective care
* Recognise, assess, and refer patients with mental health needs or safeguarding concerns
* Contribute to achievement of QOF targets, clinical audits, and PCN service delivery
* Maintain accurate, up-to-date clinical records in line with NMC and NHS standards
* Work collaboratively within the multidisciplinary team to support clinical emergencies and vulnerable patients

Essential Requirements
* NMC Registered Nurse (Level 1)
* Advanced Nurse Practitioner qualification (Master's level or equivalent)
* Independent Prescriber qualification
* At least 1 year of experience in a GP or primary care setting
* Experience in managing both acute and long-term conditions
* Excellent communication and decision-making skills
* Full UK driving licence and access to own transport
