The post holder will assist in the overall delivery of care as determined by the Healthcare co-ordinator team and/or Nurse in Charge; to support the management, lead, and co-ordinate the implementation of the care coordination model within HMP YOI ISIS, ensuring prescribed care is carried out according to the agreed care plans in conjunction with the multi-disciplinary team. The post holder will support qualified staff in all activities relating to the delivery of high-quality patient-centred care. The post holder will work and act as a key member of the Healthcare co-ordinator/EDIC team, supporting patients in the achievement of core clinical outcomes.
Responsibilities
* To work as directed by a registered nurse and/or the Team Leader.
* To provide care physical care to patients in accordance with their care plan
* Understanding the need for and always maintaining confidentiality and security.
* Adhering to all organisational and prison policies, procedures, and guidelines.
* Ensuring good working relationships with offenders, carers, relatives, other professionals and prison and healthcare staff
* Developing an understanding of and demonstrating use of clinical supervision to reflect upon and modify practice.
