Overview
Community Connectors work as part of an integrated, blended mental health offer to support people for approximately a 6 - 12 week duration, with emotional, social and practical needs to access a range of local, non-medical and non-clinical services, to improve confidence, social inclusion and independence. This role will involve working as part of a multi-disciplinary team with statutory, third sector and NHS colleagues to support clients who are suffering from moderate to severe mental health difficulties. You will need to have experience working with those suffering with mental health illness and demonstrate in-depth knowledge of the services, support networks and local resources that may assist in improved outcomes for those dealing with complex and enduring mental health issues. The ability to be adaptable, work independently and the resilience to work in challenging environments are key attributes for this role.
Responsibilities
* Attend MDT (multidisciplinary team) meetings to discuss patients, including those currently on mental health wards and individuals under a section; discuss high-risk cases and next steps, and those ready for discharge.
* Record notes from meetings in the database, reporting on the discussion and outcomes for patients; identify next steps for professionals and tasks for the admin team.
* Conduct initial assessments with new service users, introduce yourself, understand goals and what recovery means to them, carry out a dialog+ diagnostic tool, clarify areas for signposting and discuss wellbeing and practical support.
* Signpost services for the service user and update the database with notes from the initial assessment; upload documents sent to services for the new service user.
* Meet with service user in a local setting, review goals and progress, review attended signposted services, assess what has worked or could be improved, and set goals for the next session (e.g., self-affirmation, attending classes, meditation, self-care tasks).
* Handle admin tasks, respond to new referrals, and call service users to book appointments or offer well-being check-ins.

* Experience working with people experiencing mental health illness and strong knowledge of local services, support networks and resources.
* Ability to adapt, work independently, and maintain resilience in challenging environments.
* Ability to work as part of a multi-disciplinary team with NHS, statutory and third-sector colleagues.
