Job overview To assess, plan and deliver physical activity and rehabilitation programmes To assess the progress of patients during their time within the stroke pathway and to adapt their programmes appropriately to ensure safe and effective exercise, physio, and rehabilitation. To promote healthy living in patients and support behaviour change. Adopt a personalised care approach to management of patients Address health inequalities through provision of service as a priority in patient care and service development. Contribute to the operational management and continuous improvement of the stroke ‘one stop’ service. Advert To frequently undertake the comprehensive assessment of patients, with a complex presentation, requiring prolonged concentration to investigate and analyse the assessment to formulate individualised management and treatment plans, using clinical reasoning and a wide range of treatment skills and options to formulate a specialised programme of care. To use continuous assessment to evaluate treatment outcomes and adjust treatment plans accordingly, appropriately referring to other agencies to meet patients’ needs and promote integration of services. To receive and communicate complex, sensitive, or contentious information to patients and carers, who may have barriers to understanding, e.g. dysphasic, complex mental health problems, to maximise rehabilitation potential and to ensure understanding of conditions. To have expert verbal and non-verbal communication skills of motivation, explanation, and reassurance to agree and gain co-operation of patients and carers in their treatment programmes. To provide spontaneous and planned advice, teaching and instruction to relatives, carers, and other professionals, to promote understanding of the aims of physiotherapy and to ensure a consistent approach to patient care and treatment e.g. moving and handling. Contribute and propose changes in the development and implementation of service changes within clinical areas and policy development.
