Overview
This post is a Community Alcohol Care Team Practitioner providing specialist bespoke, intense care and support to change resistant drinkers, frequent hospital attenders and the more complex patients who have not engaged with other services to support them in the community settings of Cwm Taf Morgannwg University Health Board (CTMUHB).
This post is a fixed Term/secondment for 12 months due to funding. If you are interested in applying for it as a secondment, please obtain permission from your current line manager prior to applying.
Community Alcohol Care Team practitioner providing nursing care to patients in the community setting.
To work in collaboration with the Mental Health, Clinical and Nursing staff across the Primary and Secondary care sectors in the provision of highly specialised care to patients with Alcohol Misuse.
To assist the Clinical Team in the delivery of any relevant aspect of patient care from diagnosis to discharge.
To provide assessment, advice, information and referral for individuals experiencing alcohol misuse problems from the general hospital setting.
Responsibilities
* Collaborate with Mental Health, Clinical and Nursing staff across Primary and Secondary care to deliver highly specialised care to patients with alcohol misuse.
* Assist the Clinical Team in delivering patient care from diagnosis to discharge.
* Provide assessment, advice, information and referral for individuals experiencing alcohol misuse problems within the general hospital setting.
* Provide nursing care to patients in community settings as part of the Community Alcohol Care Team.
* Work to engage change-resistant drinkers and frequent hospital attenders who have not engaged with other services.
Qualifications and Skills
* Welsh Skills Desirable: This post is advertised as Welsh Desirable. Not essential, but considered advantageous in shortlisting. Level 3 Speaking & Listening (equivalent to CEFR B2) or above; basic conversations with patients about everyday health.
* Experience in community nursing or care for patients with alcohol misuse is desirable.
* Ability to work collaboratively across multidisciplinary teams and to coordinate care from diagnosis to discharge.
