Responsibilities
* Provide clinical leadership and line management for a Community Nursing team, working collaboratively with operational leads.
* Assess new and complex patients in their homes, plan and evaluate care, and deliver the Enhanced Health in Care Homes Framework in line with local objectives.
* Lead continuous quality improvement, support clinical governance, and manage caseload processes within your team.
* Help patients and carers achieve optimal independence using best practice and adhering to national and Trust guidelines.
* Develop and deliver a 7‑day consistently high‑quality, safe, sustainable, and cost‑efficient service operating 08:00 to 18:00.
Qualifications
* District Nurse who holds a Community Specialist Practitioner Qualification with significant Community Nursing experience.
* Demonstrate a comprehensive specialist knowledge base and promote excellence in nursing practice in a dynamic healthcare environment.
