Job overview

A fantastic and exciting opportunity has arisen within Whittington Health NHS Trust for an experienced and highly motivated Band 8a Clinical CHC Team Lead.

The post will cover all aspects of Continuing Healthcare provision in Haringey and Islington consisting of patients in the community and provide clinical and nursing leadership and oversight of operational management for each area. The post provides an invaluable opportunity for a nurse to experience working across community services and makes this an ideal opportunity for applicants who are looking to further their clinical, service development and leadership skills in the NHS.

The post is full-time, 37.5 hours per week Monday - Friday 9-5pm. We also have flexible working policies to support individuals achieve the work – life balance that their situation might require.

1. The post holder will manage a team of staff in Haringey and Islington boroughs providing excellent management and professional leadership to the Continuing Healthcare team.
2. The post holder will be responsible for ensuring the statutory requirements as laid out in the National Framework for NHS Continuing Healthcare (CHC) and NHS - Funded Nursing Care (FNC) are implemented.
3. The post holder will develop robust processes and procedures and policies which are compliant with legislative and national guidance and in accordance with ICB responsibilities.

Main duties of the job

4. To be responsible for ensuring that all CHC processes implemented are compliant with legislative and national guidance and in accordance with the responsibilities of the ICBs.
5. To work with Mental Health Trust and LA partners for Section 117 of the Mental Health Act in relation to after care, in the context of continuing care.
6. To maintain a robust decision making process including for the ratification of MDT recommendation and where there is a panel, to monitor its running and processes are in accordance to the CHC Framework for Older People, Physical Disabilities & Complex Needs, Mental Health and Learning Disabilities.
7. To manage contentious situations in relation to disputes between partner agencies or with individual patients and relatives.
8. To monitor the quality of care packages for continuing healthcare and funded nursing care through reviews and case management.
9. To have lead responsibility for establishing and coordinating arrangements across a range of agencies including NHS providers, third sector and independent sector providers.

10. To hold lead responsibility for delivery of the action plans relating to continuing healthcare, including leading service reviews, developing business cases and procuring services to meet identified gaps.

11. Responsible for the management of all staff within the Islington and Haringey CHC team.

Detailed job description and main responsibilities

12. To be responsible for ensuring that all CHC processes implemented are compliant with legislative and national guidance and in accordance with the responsibilities of the ICBs.
13. To work in partnership with the Local Authority, Acute Trusts and Care Homes to ensure timely discharges and good quality and cost-effective placements.
14. To implement a rolling programme of training for all continuing healthcare staff as well as partner organisations to achieve ongoing improvements in the system and keep abreast and disseminate policy changes.
15. To prepare cases and reports in relation to any challenges to the established decision-making process, including supporting ICBs in instructing solicitors as required, up to and including judicial reviews.
16. To work with the ICB Management, Head of Adult Social Care, Older People, Acute Hospital Discharge Management, Mental Health Teams, Out of Boroughs ICBs and Acute Hospitals to support the achievement of the Local Area Agreement target in relation to delayed transfers of care.
17. Monitor staff performance and implement corrective action when required i.e. disciplinary procedures, sickness absence management, and grievance procedures.

22. Comprehensive understanding and experience of the CHC National Framework for NHS continuing healthcare and NHS funded Nursing Care
23. Budgetary management experience and understanding of the current NHS financial regime
24. Knowledge of a wide range of staff management practices including: IPR, Disciplinary process, Recruitment, performance, redeployment and absence management
25. Evidence of significant management experience, communicating at all levels of the organisation.
26. Evidence of working with the judicial system
27. Experience of staff management including: managing recruitment, performance monitoring, and appraisal, induction and mentoring.
28. Experience of system leadership
29. Experience of chairing multi- disciplinary meetings including in a multiagency setting.

31. Evidence of close working relationships with NHS Commissioners
32. Knowledge and understanding of key Govt. legislation, national guidance / drivers relating to CHC
33. Knowledge of wide range of senior management practices including budgetary management, the consultation process, workforce planning cost pressures and service developments
34. Understanding E of clinical risk and clinical governance principles and their application
35. Understanding Adult Safeguarding and DOLs statutory requirements and procedures.
