About the Role

You will support individuals and carers facing complex challenges such as ageing, mental health needs, disability, dementia, and social isolation—while also guiding and developing colleagues.
You will play a vital role in triaging referrals, assessing risk and urgency, and ensuring timely, high-quality interventions. You’ll also contribute to shaping services, strengthening multi-agency relationships, and ensuring best practice across the team.

This is a hybrid role, with at least 40% of your time based in one of our community settings.
Key Responsibilities

* Work with senior leadership to develop and continuously improve services
* Lead and promote multi-disciplinary and multi-agency collaboration
* Build strong networks across statutory, voluntary, and community services
* Support colleagues to identify services that promote independence, choice, and dignity
* Ensure timely and accurate recording of all interventions on the CRM system
* Contribute to practice development and innovation within the team
* Represent Jewish Care in professional forums and community settings
* Maintain an up-to-date CPD portfolio and support others in their development
