Main duties of the job
EastleighSouthern Parishes Network Ltd is a local GP federation responsible for managingthe Primary Care Network services and staff. It is an ambitious organisationowned by 2 local GP practices and a local management consultancy. We believethat through sharing our expertise we can provide improved services to ourpatients as well as strengthening the primary care infrastructure. We believewe have a very exciting and forward-thinking organisation which is committed tosignificantly delivering improvement to the quality of patient care. With thegovernment-lead PCN agenda we now find ourselves requiring an advanced nursepractitioner, who shares our values and has a desire to be an excellentprovider of care, to join our existing highly skilled team.
Candidateswill be employed on a pay rata based on the equivalent of Band 7-8 (although weare not an agenda for change organisation) depending on qualifications andexperience. We encourage personal clinicalprogression in a nurtured environment according to need.

Job responsibilities
Towork autonomously and across the primary care network practices BlackthornHealth Centre, Hedge End Surgery and ESP Primary Care Hub in Lowford, usingadvanced knowledge and skills, underpinned by theory and experience.
Assessand triage patients, including same day triage, and as appropriate providedefinitive treatment (including prescribing medications following policy,patient group directives, NICE (national) and local clinical guidelines andlocal care pathways) or make necessary referrals to other members of theprimary care team.
The post holder is expected to beable to:
Works unsupervised,clinically examining and assessing patients and initiating any appropriateaction.
Workingat a masters level or equivalent that encompasses the four pillars of clinicalpractice, leadership and management, education and research, with demonstrationof core capabilities and area specific clinical competencies
Manageundifferentiated undiagnosed condition and identify red flags and underlyingserious pathology and take appropriate action
Usecomplex decision making to inform the diagnosis, investigation, completemanagement of episodes of care within a broad scope of practice, actively takea personalised care approach and population centred care approach to enableshared decision making with the presenting person, and complete the relevanttraining in order to provide multi-professional clinical practice and CPDsupervision to other roles within primary care, for example first contactpractitioners and the personalised care roles.

1. Candidates must have either graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (supported) Route; both pathways enable eligibility for an Advanced digital badge issued by the Centre, demonstrating recognised educational and experiential preparation in advanced practice.
2. NMC Registered Nurse with clinical post registration experience.
3. Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet.
4. Health Assessment / minor illness qualification.
