Are you an experienced and passionate Social Work professional with strong leadership skills and a background in Learning Disabilities and Transitions? We're working with a well-established local authorityin Greater Manchester to recruit an Assistant Team Manager (ATM) to lead on Transitions within the Learning Disability Service.

About the Role
As the ATM / Transitions Lead, you’ll play a crucial role in overseeing complex cases and ensuring smooth, well-coordinated transitions from Children’s to Adults’ Services for individuals with learning disabilities.
You’ll lead by example—supporting practitioners, championing best practice, and working alongside partner agencies including education, health, and housing to achieve positive long-term outcomes.
Key responsibilities include:
1. Providing leadership and operational support to the Learning Disability Social Work team
2. Coordinating and managing Transitions cases (16–25 age group)
3. Ensuring compliance with relevant legislation including Care Act 2014 and Children and Families Act 2014
4. Supporting and supervising qualified social workers and contributing to reflective supervision
5. Overseeing Care Act and Mental Capacity assessments
6. Leading multi-agency planning meetings and safeguarding responses
7. Driving forward a strengths-based, person-centred approach across all interventions
8. Ensuring smooth pathways into Adult Social Care services and support options
You'll work closely with Team Managers and Senior Leaders, contributing to service development, quality assurance, and innovation within the Learning Disability service area.
About You
We're looking for a confident and capable leader with a passion for delivering excellent services for young people and adults with learning disabilities.
You must have:
9. A recognised Social Work qualification (SW Degree, DipSW, CQSW or equivalent)
10. Registration with Social Work England
11. Minimum 4 years’ post-qualification experience in Adult Social Care (Learning Disability or Transitions preferred)
12. Proven experience managing or supervising social workers or complex cases
13. Strong understanding of relevant legislation, including:Care Act 2014Mental Capacity Act 2005Children and Families Act 2014Mental Health Act
14. Excellent communication and organisational skills
15. Ability to build strong, productive relationships with service users, families, and professional networks
16. A proactive, reflective, and solutions-focused approach to leadership and practice
