Looking to step into a role where your Case Management expertise creates real impact for older Australians wanting to stay independent at home?
We're seeking an experienced Support at Home Case Manager to join a small, supportive team in Ulverstone. This is a full-time locum contract with an inital 6 month duration
What You'll Be Doing
Managing a caseload of approximately 45 clients
Coordinating care plans and providing individualised support
Onboarding new referrals and conducting assessments
Using MAC (My Aged Care) confidently to manage client journeys
Working collaboratively within a close-knit team environment
What You'll Bring
Solid Case Management experience within aged care, community care or home care
Strong MAC platform knowledge and Support at Home knowledge
Experience onboarding and managing new clients
A client-first approach with excellent communication skills
* Nursing background highly regarded
Location: Ulverstone (office + community) with flexible work options available to reduce travel/office days if coming from Burnie/Launceston/Devonport etc - offering 2-3 days per week remote working, with at least 2 days in the office.
Start: 05/01/26
Hours: Full-time or 4 dpw
