What You’ll Do: Assess and support older adults with complex care needs, developing personalised plans to promote independence and well-being. Collaborate with families, healthcare providers, and community resources to deliver tailored interventions. Maintain accurate records, adhere to safeguarding standards, and travel across Powys as needed (e.g., 15 minutes to Builth Wells, 42 minutes to Brecon). Work 37 hours weekly, making a positive impact in a supportive team environment. What We’re Looking For: A qualified Social Worker with experience in adult services, specialising in older people, and registered with Social Care Wales. Strong assessment and case management skills, with a person-centred approach and flexibility to travel. Ability to work full-time (37 hours) and reside within a reasonable commuting distance to Llandrindod Wells, LD1 5LG. Essential Requirements: Eligibility to work in the UK.
