Description Assistant Practitioner - Preparing for Adult Life Service | 10306 | permanent contract | 37 hours per week | £32,061 to £33,699 (G grade) or £29,064 to £31,022 (F grade) per annum | Norwich The Preparing for Adult Life (PfAL) Service ensures young people moving into Adult Social Care are supported to do so in a seamless, joined up way. You will have the ability to work with young people with complex needs (including those with Learning Disability, complex mental health needs or physical disabilities, young people who are Autistic), as well as their families. You will be able to demonstrate an understanding of the legislative requirements in relation to the Care Act 2014, as well as other awareness of other legislation including the Children and Families Act 2014 and Mental Capacity Act. As part of this role you will be expected to carry out assessments and care planning under the Care Act 2014. To build meaningful relationships with the people we support, this role requires regular in-person work in both the office and community. We're looking to appoint either a Supported Assistant Practitioner (Scale F) or an experienced Assistant Practitioner (Scale G) and require a CV and Supporting Statement from you.
