Job Purpose:
To provide practical, emotional, and person-centred support to adults with disabilities, mental health needs, learning difficulties, or age-related conditions, enabling them to live independently and safely within their own homes and communities. The role focuses on empowering individuals to maintain or develop life skills, access local services, and improve quality of life.
Key Responsibilities:

Support adults with daily living tasks including personal care, meal preparation, household chores, and medication prompts.

Encourage and enable individuals to develop independent living skills such as cooking, cleaning, budgeting, and personal hygiene.

Assist service users in accessing the community for shopping, social activities, education, volunteering, or employment opportunities.
*
Provide companionship and emotional support to reduce isolation and promote wellbeing.
*
Collaborate with families, carers, health professionals, and social workers to deliver holistic and coordinated support.
*
Follow personalised care/support plans and contribute to regular reviews and assessments.
*
Maintain accurate records and complete reports on progress, concerns, incidents, or changes in need.

Monitor the safety and wellbeing of service users and report safeguarding concerns promptly.
*
Promote dignity, choice, and independence in line with person-centred values and legislation such as the Care Act 2014.
Person Specification:
Essential:
*
Experience working with adults with support needs (e.g., disability, mental health, ageing, substance misuse).
*
Knowledge of independent living principles and safeguarding adults at risk.
*
Good communication and interpersonal skills.
*
Ability to work alone and as part of a team.
*
Flexible and responsive to the needs of service users, including evenings and weekends.
*
Enhanced DBS check (or willingness to undergo one).
Desirable:

Relevant qualifications (e.g., NVQ Level 2 or 3 in Health and Social Care).

Full UK driving licence and access to a vehicle (if role involves community visits).
*
Understanding of the Care Act, Mental Capacity Act, and other relevant legislation.
