Overview
We are looking to recruit a qualified Band 4 Assistant Practitioner to join our Crisis Resolution Home Treatment (CRHTT) team based at Gt Yarmouth acute services, Northgate Hospital.

Responsibilities
* Assessments, risk management, care planning, and co-ordination of care delivery. You will support the Registered Nurses, Social Worker and Occupational Therapists with day-to-day activities and provide support to service users throughout their recovery journey.
* Work alongside Clinical Support Workers and Registered Nurses, with a combination of shifts including unsociable hours, some weekends and bank holidays.
* Be part of a well-resourced team, supported with regular supervision and opportunities to work in partnership with health and social care in the wider multi-disciplinary service. Contribute to the development of the role of an Assistant Practitioner and participate in change management projects such as quality improvement.
