The Community Children’s Nursing Service is seeking to appoint an enthusiastic and highly motivated individual to contribute to the established Children’s Hospital at Home service.
The Children’s Hospital At Home Service Is An Exciting, Dynamic, Ever Developing Team That Was Set Up In 2021 And Now Offers a Number Of Clinical Pathways In Which To Support Children In The Community Including
* Bronchiolitis
* Viral Induced Wheeze
* Jaundice
* IVAB
* Gastroenteritis
* Non pathway referrals
The successful candidate will work within the Children’s Hospital at Home team in delivering acute nursing interventions to the children and young people on our Hospital at Home caseload. The role involves undertaking planned clinical assessments following set care pathways and escalating appropriately. The Children's Hospital at Home Team Nurse will be supported by the acute paediatricians and Lead Nurse for Hospital at Home, ensuring that children are kept safe at home whilst on the caseload.
The successful candidate will be able to support incident reporting, service development and Quality Improvement and Research opportunities.
The Children’s Hospital at Home Team Nurse will work within the Community Children’s Nursing Service and will support the caseload of complex patients when required. The Children’s Hospital at Home Team Nurse will support the overall service delivery by contributing to the end of life on-call rota which is activated when required. Training and support will be provided.
The Children’s Community Nursing Team offer a rolling education programme, including a full induction for new starters into the team.
