Assertive Outreach Senior Outreach Worker / Care Coordinator
MAIN DUTIES / RESPONSIBILITIES:
1. Communication & relationship skills: The post-holder will work with all members of staff within PCN's and Navigo, collaborating closely with health and social care professionals in statutory and non-statutory agencies to facilitate and deliver services that meet the continuing care needs of service users. They will participate in multidisciplinary meetings, case conferences, and other relevant meetings. Responsibilities include providing and presenting reports at Mental Health Review Tribunals and Hospital managers hearings. The role involves effectively communicating complex and/or sensitive information, especially concerning adult protection, abuse, cognitive impairment, and other risks related to a service user's mental health, often working with individuals who may be hostile, emotional, or vulnerable. Maintaining accurate clinical records, participating in supervision, and promoting innovative approaches to working with service users are essential. The post also requires developing relationships with service users and/or carers to ensure accurate care delivery and managing confidential information in accordance with policies. The role includes working with service users subject to restrictive interventions such as Community Treatment Orders and DOLs.
2. Knowledge, training, and experience: Attend relevant training identified through supervision, maintain and update skills relevant to the role, participate in team and clinical meetings, and engage in clinical supervision, appraisals, and performance reviews. Contribute to training and induction of new team members and other agencies. Demonstrate high standards of care, participate in audits/surveys, and apply specialist knowledge related to older people and mental health in delivering care. Provide advice and support to other agencies on mental health issues.
3. Analytical & judgement skills: Under supervision, make treatment and care decisions based on assessments of service users' needs.
4. Planning & organisational skills: Prioritise and coordinate own workload, including monitoring medication, scheduling appointments, conducting assessments, managing emergencies, and supporting colleagues. Work independently, seek assistance when needed, and participate in caseload management.
5. Physical skills: Be trained in control and restraint techniques, including regular updates (RESPECT).
6. Responsibilities for patient/service user care: Review clinical risks, provide health education and advice, initiate carer support, and participate in clinical governance activities.
7. Responsibilities for policy & service development: Adhere to policies and legislation, contribute to service evaluation and development, and suggest improvements.
8. Responsibilities for resources: Use equipment responsibly within the team budget.
9. Responsibilities for human resources: Support clinical supervision, mentor students, and assist with induction of new staff.
