Older adult dementia rapid response team

Derby
Derbyshire Healthcare NHS Foundation Trust
£25,000 - £40,000 a year
Posted: 1 October
Offer description

We have a fabulous opportunity for a Band 5 Registered Mental Health Nurse to join our Dementia Rapid Response Team. Applications are welcome from newly qualified nurses and students in their final year who are soon to qualify.

We cover the Derby City and South Derbyshire area.

You will work closely with Cubley Court, the multidisciplinary team (MDT), and the Discharge team to ensure a smooth and safe transition for patients from the ward environment to community settings, such as the patient's own home or a care home. You will also maintain close links with Community Mental Health Teams, GPs, Social Care, and Care Homes.

The successful candidate will employ a wide range of skills, building on and developing their expertise in managing complex cases as part of a wider multidisciplinary team, which includes Clinical Lead, Occupational Therapists, Nurses, Dementia Specialist Support Workers, Psychologists, and a Consultant Psychiatrist.

The ideal candidate should be enthusiastic, resourceful, caring, and possess excellent communication skills. They will need to demonstrate an awareness of mental health and knowledge of working with older adults.

In return, we offer a full induction, developing your knowledge, skills, and confidence in all aspects of the post.

Preceptorship is available for newly qualified members of staff.

You will play a pivotal role within dementia care, bridging the gap between medical interventions and person-centred support for individuals living with dementia. This position demands clinical competence, empathy, adaptability, and teamwork.

Your responsibilities include conducting initial and ongoing holistic assessments of patients, gathering detailed health histories—covering mental status, cognitive abilities, physical health, social circumstances, and risk factors. Collaboration with families and carers ensures all perspectives are considered throughout the care process.

You will design individualised care plans tailored to each patient's needs, abilities, and preferences, regularly reviewing and adjusting these as conditions change. Risk management strategies must be implemented and assessments updated, with concerns escalated as necessary.

Maintaining detailed records of patient care, interventions, incidents, and outcomes is essential, with confidentiality upheld at all times.

The team is friendly and supportive as well as dedicated to providing excellent care and service through evidence-based interventions, research, and quality improvement work.

You will receive regular supervision and will be well supported by the wider team. Individual staff development and CPD opportunities will be offered and reviewed as part of a robust Appraisal.

Join 'Team Derbyshire Healthcare' and become part of a talented, compassionate and enthusiastic workforce committed to a vision of 'making a positive difference in people's lives'.
