We're recruiting an Outreach Family Support Worker to deliver high quality, home based Early Help support to children, young people and their families. You'll hold a caseload, provide targeted interventions, and coordinate multi agency support using a whole family, strengths based approach. The role focuses on preventing escalation to statutory services and improving outcomes for children aged 0-25 (19-25 where SEND applies). Key Responsibilities Hold and manage a caseload of children and families with multiple or complex needs. Complete Family Help assessments and lead Team Around the Family (TAF) meetings. Provide intensive, targeted support to vulnerable children and young people, including those with emotional or mental health needs. Develop and coordinate multi agency plans that reduce risk and improve outcomes. Maintain accurate, timely case records, assessments, plans and chronologies. Key Requirements Experience working with children, young people and families with complex needs. Strong assessment, planning and case holding skills. Ability to lead multi agency meetings and coordinate support. Excellent communication, organisation and report writing abilities. Knowledge of safeguarding legislation and Early Help frameworks.
