The Swan Medical Centre, Located in South Yardley Birmingham, has an Exciting opportunity to join a happy, democratic, 4 partner practice

Caring for 8,000 patients in a modern, purpose-built building.

We have a fantastic team which include the GP Partners & SalariedGPs, 3 Clinical Pharmacists, 1 Paramedic, 2 Practice Nurses and 1 HCA. The non-clinical team consists of Practice Manager, Deputy Manager, 3 administrators, 2 secretaries and 5 reception staff.

We are happy to support the development of special interests.

1 position is available. (Sessions to be agreed)

Indemnity paid (pro rata)

6 weeks holiday and 1 week study leave (pro rata) and BMA model contract

Main duties of the job

The post-holder will manage their own patient caseload and provide high-quality care across a wide range of primary care needs, for both registered and temporary patients.

Duties will include:

Conducting face-to-face and telephone consultations in line with the agreed practice timetable.

Visiting patients at home when required.

Managing prescriptions and responding to queries, correspondence, and paperwork promptly.

Assessing and managing patients with undifferentiated or undiagnosed problems.

Screening patients for disease risk factors and early signs of illness.

Developing personalised care plans in consultation with patients, in line with current disease management protocols.

Maintaining accurate, contemporaneous consultation notes to agreed standards.

Issuing and managing computer-generated acute and repeat prescriptions, adhering to the practice formulary and prescribing guidelines.

The post-holder will be expected to undertake all standard duties of a GP in primary care, contributing to the smooth running of the practice and delivering excellent patient care.


About us

Our practice was founded in 1905 by Dr Hugh Donovan, who established his surgery at 1466 Coventry Road in a spacious Victorian house with an adjoining coach house. At the time, Yardley was a growing community just beyond Birmingham, and Dr Donovan quickly became a familiar figure, often making his rounds on a motorbike with a sidecar.

Following Dr Donovans retirement around the outbreak of the Second World War, his sons carried on the practice, maintaining the high standards he set. Today, our team of four partners continues to uphold these exceptional standards, combining a strong tradition of family medicine with a forward-looking approach to patient care.

Primary care is evolving rapidly, and meeting the changing needs of our patients requires fresh ideas and innovative solutions. While we embrace these new challenges, our commitment remains firmly rooted in the core ideals of family medicine: compassionate, personalised care for every patient.


Job responsibilities

Provide general medical services to registered and temporary patients across a wide range of primary care needs.

Conduct face-to-face and telephone consultations, home visits, and other patient care activities in accordance with the practice timetable.

Assess, diagnose, and manage patients with undifferentiated or undiagnosed problems.

Screen patients for disease risk factors and early signs of illness.

Develop and implement personalised care plans in consultation with patients, following evidence-based guidelines and practice protocols.

Maintain accurate, contemporaneous medical records and consultation notes to agreed standards.

Prescribe and manage acute and repeat medications, adhering to the practice formulary and current prescribing guidelines.

Respond promptly to patient queries, correspondence, and administrative tasks.

Participate in regular clinical meetings, audit, and quality improvement initiatives.

Undertake all other duties expected of a GP working in a primary care setting.


Person Specification


Qualifications

* GMC-registered with a licence to practise in the UK.
* Completion of GP training (CCT or equivalent).
* Ability to manage a varied caseload in a busy primary care setting.
* Excellent communication, organisational, and interpersonal skills.
* Commitment to providing patient-centred care.
* Experience in [specific areas, e.g., chronic disease management, minor surgery, teaching].
* Interest in quality improvement, leadership, or education.
