Main duties of the job
In this role, you'll deliver a broad range of patient care services, supporting individuals with both routine and long‑term health needs. You'll play a key part in promoting health and wellbeing, carrying out clinics and providing advice that empowers patients to manage their own health.
Particular focus is on long‑term condition management such as diabetes, hypertension, asthma, COPD, heart failure and dementia — working closely with the GPs and the pharmacist to optimise patient care.
Leading on ladies' health screening including cervical smears and swabs and childhood immunisations is also an important aspect of the role.
Working closely with the clinical team, you'll help ensure safe, effective, and patient‑focused care, while also contributing to the smooth day‑to‑day running of the practice. You'll be involved in maintaining high clinical standards, accurate record‑keeping, audits, and supporting a positive experience for patients at every interaction.

Responsibilities
* Scheduled and unscheduled childhood immunisations
* Cervical smears and swabs
* Chronic disease management – asthma, COPD, diabetes
* Health promotion
* Measuring blood pressure
* Basic wound care and dressings
* Contraception
* Perform ECGs and venepuncture when required
* Monitor and manage maintenance of stock and equipment, including refrigeration and emergency equipment
* Provide care inline with NICE guidelines
* Contribute towards clinical targets included in QOF and QIF
* Communicate effectively with other team members
* Communicate effectively with patients and carers
* Infection control lead
* Frailty and dementia reviews
* Carry out home visits for nursing assessments/reviews when required
Education & Development
Participate in continued education and maintain an up‑to‑date level of professional knowledge and skills. Identify personal development and training needs with the manager. Support junior staff and assess their competency against set standards. Record information and activities undertaken with patients and carers in an accurate and timely fashion using EMIS. Maintain confidentiality of information relating to patients, carers, relatives, staff and the practice. Take the required precautions when providing information, including following GDPR. Create and run audits relating to own area of work, e.g. cervical screening, infection control.
Health and Safety
The post‑holder will help to promote and maintain their own and others' health, safety and security. Using appropriate infection control procedures, maintain work areas in a tidy and safe way and free from hazards. Report potential risks that are identified. Undertake infection control training.
Qualifications (Essential)
* Qualified registered nurse (Nursing and Midwifery Council) alongside evidence of continued professional development
* Disclosure and Barring Service Check (Rehabilitation of Offenders Act (Exceptions Order) 1975)
Experience
* Primary Care 1 year (preferred)
* Knowledge of clinical governance and CQC requirements
* Organisational skills able to prioritise workload
* Experience of dealing with the public/patients
* Experience of dealing with confidential information
