HCRG Care Group is seeking a dedicated Heart Failure Specialist Practitioner (HFSP) to join our dynamic Heart Failure Service, supporting patients with a diagnosis of heart failure across our community.Our Heart Failure Service is committed to:Enhancing the quality of life and end-of-life care for patients living with heart failureMinimising symptoms through achieving maximum medical therapyReducing mortality and morbidity through proactive, evidence-based interventionsAs an HFSP, you will play a pivotal role in assessing individual patient needs and developing holistic, personalised treatment plans in collaboration with General Practitioners. You will use national and local guidelines and protocols to ensure best practice and continuity of care.Key responsibilities include:Delivering advanced nursing care and therapeutic interventionsSupporting patients to achieve improved prognosis, symptom control, and overall wellbeingAdvocating for patient-centred care across the multidisciplinary teamFor further information or an informal chat please contact Lisa Bailey -lisa.bailey@hcrgcaregroup.comPackage DescriptionAs a Heart Failure Specialist Practitioner /Nurse - Band 6 at HCRG Care Group, we value you and your contributions, offering a range of benefits to support your professional growth and personal wellbeing:• Competitive Pay & Pension: Receive a salary of £37,338 - £44,962 with Agenda for change terms and conditions including NHS pension. • Professional Growth: Join our Strive for Better networks, connecting with professionals nationwide to develop and share best practices. • Exclusive Rewards: Access discounts and offers at leading supermarkets, retailers, and experience providers through our Reward Gateway. • Wellbeing Support: Benefit from our 24/7 free wellbeing service, including counseling, career coaching, and legal advice. • Flexible Pay Options: Track earnings and access wages through Wagestream, providing financial flexibility. • Learning & Development: Enhance your career with support from our Learning and Development team, access e-learning, career pathways, and funding opportunities. • Recognition & Involvement: Celebrate achievements with local and national awards, contribute ideas for service improvements, and stay informed through regular leadership updates.Main ResponsibilitiesThis post is responsible for • To identify high risk patients/ type of patients eligible for interventions provided by the HFS. .To ensure own knowledge of appropriate NMC standards and HCRG professional guidelines and that these are applied to practice. .• To be able to produce heart failure treatment plans according to national and local guidelines following interpretation of echo results and clinical history. • Review patients both in practitioner-led community heart failure clinics and provide support through home visits for housebound patients. • Working as a part of a team, be responsible for the education of patients with heart failure and their families. • Working with staff in secondary care, ensuring effective discharge planning for heart failure patients making best use of resources, utilising best practice and optimising health outcomes To foster good working relationships with other health professionals to ensure patients receive optimal care by developing and promoting policies and protocols which encourage a unified approach to heart failure in the local health community. • To provide an effective interface between primary and secondary care thereby improving the management of heart failure. • To provide professional help, support and education to patients and their relatives, which will enable them to effectively manage their illness.Please see the attached job description for the full list of responsibilities and specification.The Ideal CandidateEssential• Registered Health Care Practitioner qualified to degree level.• Expert knowledge in the treatment and management of Heart Failure.• Knowledge of pharmacology in the management of long-term disease.• Experience working in a multi-disciplinary team and providing care to patients with heart failure• Strong interpersonal, written, and oral communication skills.• IT literacy and ability to maintain accurate clinical records.• Ability to work independently and flexibly in response to service needs.• Commitment to ongoing professional development and service improvement.Desirable• Further qualification in Heart Failure treatment or equivalent knowledge and experience.• Non-medical prescribing qualification or willingness to work towards it• Experience in community healthcare services.• Teaching experience and ability to provide education to patients and colleagues.• Experience in service development and implementation of innovative care strategies.Other Requirements• Willingness to work across different Trust areas as required.• Valid UK driving license with access to a vehicle for work purposes.• Flexible availability, including evenings, weekends, and Bank Holidays, to meet service demands.Please see the attached Job Description for the full Person SpecificationAbout The CompanyWe change lives by transforming health and care.
