The Support Worker will play a key role in delivering high-quality, person-centred care to adults with learning disabilities. This role involves working closely with internal teams, families, and external professionals to ensure that individuals are supported to lead fulfilling and independent lives. You will be involved in the development and implementation of tailored care plans, ensuring that the needs of the people we support are met in line with regulatory and organisational standards.

About the Role – Support Worker
As a Support Worker at The Bridge, you will play a vital role in empowering adults with learning disabilities, autism, and complex needs to live as independently and meaningfully as possible. You’ll work under the guidance of a Senior Support Worker or Team Leader to deliver person-centered, compassionate care that promotes dignity, inclusion, and wellbeing.

Key Responsibilities
Person-Centered Support
* Build positive, respectful relationships and involve individuals in decisions about their care.
* Provide personal care (e.g., hygiene, dressing, mobility) with dignity and respect.
* Support daily living activities, meal preparation, and community involvement.
* Encourage participation in meaningful and recreational activities.
* Monitor and report changes in health and wellbeing to the senior team.
Delivering High Standards
* Follow individual care plans and risk assessments.
* Administer and record medication in line with training and policy.
* Support individuals with healthcare appointments and medical needs.
* Promote independence and skill development.
Teamwork and Communication
* Maintain clear and accurate care records.
* Work collaboratively with colleagues, families, and professionals.
* Respect confidentiality and share information appropriately.
* Contribute to a positive and supportive team environment.
Health and Safety
* Follow all health and safety procedures to ensure a safe environment.
* Report any safeguarding concerns, incidents, or hazards.
* Maintain awareness of infection control and COSHH regulations (as outlined in our QCS Safety Management booklet).
Professionalism and Compliance
* Communicate respectfully and represent The Bridge in a professional manner.
* Adhere to safeguarding, data protection, and whistleblowing policies.
* Follow all policies and procedures, including those on QCS and in the Right Way Handbook.
* Uphold professional boundaries at all times.

Training and Qualifications
* All Support Workers must have or be working towards a Level 2 Diploma in Health & Social Care. This is essential for your role.
* Training and development support will be provided to help you succeed and grow.
