This is a full‑time Band 5 community staff nurse vacancy within the City District Nursing Team.
The team supports patients registered with the 2 Primary Care Networks in the City of Herefordshire. With a caseload of around 850 patients, staff receive supervision for competency achievement.
Our base is at Belmont and we share a large office with neighbourhood team therapists, which enables excellent communication regarding shared patient care.
Main duties of the job
Most of our work is with housebound patients, but we also run a District Nurse Clinic for non‑housebound patients who need the specific skills of a District Nurse to support their treatment.
The focus of District Nursing is to support patients towards self‑management, enabling them to continue to live as fully and productively as possible, through education, equipment and lifestyle changes. We also have the privilege of supporting people and their families at the end of their lives, working in close liaison with Hospice Teams and Specialist Palliative Care Nurses.
As a community team we work with many other health and social care professionals, Care Homes and Care Agencies. All staff are expected to record their work electronically on EMIS, our electronic patient record system which links into the GP services to allow seamless service provision. Training and ongoing support will be provided for EMIS and other electronic systems.

* Evidence of continued professional development
* Must be able to demonstrate an understanding of confidentiality in all of the role
* IT literate (including Microsoft Office) and willing to learn new systems
* Good communication skills both written and verbal with patients and across the MDT
* To be able to plan and deliver person‑centred care
Experience
* Working in a range of clinical settings

£32,073 to £39,043 a year (pro rata) – plus enhancements for unsocial hours.
