You will play a pivotal role in delivering high-quality urgent care, supporting admission avoidance, and facilitating timely discharges/referrals. Working closely with a multidisciplinary team, you will contribute to innovative patient pathways and service improvement., As a Trainee ACP, you will develop the skills to independently assess, diagnose, and manage patients with undifferentiated presentations across urgent and emergency care settings. Under supervision, you will undertake comprehensive assessments, request and interpret diagnostics, initiate treatment plans, and perform clinical procedures within your scope. You'll be supported to recognise and respond to deteriorating patients, contribute to safe discharge planning, and apply evidence-based practice. You will work closely with the multidisciplinary team, contributing to patient flow, safety, and shared decision-making. As your competence grows, you'll provide clinical advice, support junior staff, and participate in handovers and departmental meetings. Leadership development includes involvement in service improvement, clinical audits, and supervision of learners. You'll engage in teaching, support education programmes, and contribute to the development of ACP competencies. This role supports your progression through the MSc in Advanced Clinical Practice, with structured supervision and competency-based development. This ensures everyone feels supported and has a sense of belonging working for UHN., Under supervision, begin to assess, diagnose, and manage patients with undifferentiated and undiagnosed presentations in urgent/emergency care. Undertake supervised clinical assessments and contribute to decision-making regarding care pathways and referrals. Request and interpret diagnostic tests with support, including bloods, ECGs, and imaging. Perform clinical procedures within scope of assessed competence and under supervision. Support initiation and review of treatment plans; prescribe in line with training, qualifications, and Trust protocols. Recognise and escalate life-threatening conditions and deteriorating patients promptly. Develop ability to provide clinical advice under guidance from senior staff. Assist in managing patient care across relevant clinical areas under supervision. Contribute to safe discharge planning or referral decisions with oversight. Apply evidence-based practice under supervision in clinical decision-making. Maintain accurate and timely documentation in patient records. Identify and escalate safeguarding concerns appropriately. Promote patient safety and support incident reporting processes. Assist in managing patient flow and departmental pressures. Communicate effectively with patients, families, and staff, with support. Develop clinical judgement to manage risk and uncertainty under supervision. Participate in multidisciplinary handovers and ward rounds. Use digital health records and clinical systems with proficiency. Participate in on-call or extended hours as required, with appropriate support. Support patient-centred care and shared decision-making. Prescribe independently only when qualified and in line with Trust policy. Work towards completion of competencies in urgent/emergency care as defined by the Trust and professional body. Assist in teaching sessions for nursing, ACP, medical, and allied health staff. Support learners in practice under supervision. Maintain personal development through CPD, reflection, and feedback. Participate in Trust education programmes and in-house training. Contribute to development of educational content with support. Assist in developing ACP competencies and induction materials.
