Care Co-Ordinator (Patient Administration)

We are looking to recruit a Patient Administration Care Co-Ordinator to join our team at Abbey House Medical Practice.

The focus of this role is to work across all administrative teams with other primary care professionals within the Practice to provide patients with administrative support and timely access to services.

This is a permanent full-time position - 37.5 hours per week Monday to Friday, working between 10.30-18.30.


Main duties of the job

We are seeking an individual who is self-motivated and enthusiastic, approachable, compassionate, organised and must be able to work as part of a team. Previous experience of working in a people-centered environment is essential.

The successful candidate must demonstrate good communication skills face to face, over the telephone and written down plus be able to actively listen to both patients and colleagues.

A good knowledge of computers is essential. Previous use of SystmOne would be an advantage however full training will be given.

Job responsibilities

* Helppatients to manage their administration needs through answering queries,making, and managing appointments, as well as appropriate signposting withinand outside of the practice.
* Provide co-ordinationand navigation for people and their carers, across health and care servicesworking closely with the Secretarial Team, Patient Support Team and other CareCo-Ordinators.
* Supportingpatients to access their medical records, granting online access and guidingpatients on using online services, such as the NHS App.
* Understanding a patientslevel of knowledge, to ensure they have good quality written or verbalinformation to help them make shared decisions about their care, health andwellbeing.
* Makedisplays for noticeboards and ensure patients have access to up-to-dateinformation available in the waiting areas to promote services and supporttheir health and wellbeing; keeping leaflets and forms replenished at bothsites.
* Updating andrefreshing patient information forms and templates as needed and storing onTeamNet, as well as replenishing stocks of patients leaflets/forms in receptionareas at both sites.
* Ensurepatient and staff pigeonholes are checked weekly, removing items over 1 monthold and following up as needed; to include contacting patients if necessary.
* Opening,distribution and processing of incoming post and e-mail and clinicalcorrespondence; actioning/coding as necessary and/or forwarding onto the mostappropriate team/clinician for further action.
* Completionof basic non-opinion forms and core elements of forms for review by the GP forsigning.
* Handling ofSubject Access Requests and Solicitor Requests.
* Raisinginvoices when required for medical services provided to patients.
* Administrationand computerised entries with regards to the entire patient registration anddeduction pathway.
* Filing patientsrecords and retrieving written patient records as requested.
* Generaladministration support to all teams; to include outgoing post and photocopying.
* Providecover for other administrative teams in the Practice when required/directed bythe Management Team.
* Act as aChaperone for patients when requested.
* Whereappropriate refer people back to other teams and healthcare professionals.
* To work collaborativelywith the general practice team to meet the needs of patients, supporting thedelivery of policy and procedures whilst managing their own workload to deliverthe practice priorities.
* Ensure thatall activities are recorded, monitored and evaluated.
* Attend allmeetings as required.
* Work inaccordance with practice policies, ensuring that all work and procedures areundertaken in accordance with issued guidelines.
* Attend anytraining courses and supervision sessions as required.
* Promote theprofessional image and services offered by the surgery.
* Any otherduties that may be required and agreed by the Doctors and Management Team


Person Specification


Qualifications

* Safeguarding certification, or willingness to complete
* Training as set out by the Personalised Care Institute, or willingness to complete
* Training in motivational coaching and interviewing


Experience

* Experience working in a people-centred environment
* Care Co-Ordinator role in primary care
* An understanding of Safeguarding issues
* Working with SystmOne


Knowledge and Skills

* Knowledge of local area and challenges faced in the community
