Overview
To be the Physiotherapy Clinical Team Lead for the Community Hospitals Therapy Team across all four sites in Lincolnshire Community Health Services (LCHS).
Responsibilities
* Provide clinical leadership and line management to Physiotherapists within the Community Hospitals Therapy Team.
* Work closely with the Occupational Therapy Clinical Team Lead to coordinate rehabilitation services across the Community Hospitals, ensuring a cohesive approach that aligns with LCHS strategic aims.
* Collaborate with the AHP Clinical Service Lead and external agencies such as Adult Social Care to integrate pathways and ensure seamless referrals and transfers of care.
* Partner with Clinical Team Leads and the Operational Team Lead for Adult Community Therapy services.
* Work regularly across the four Community Hospital sites: John Coupland Hospital (Gainsborough), Louth Community Hospital, Johnson Community Hospital (Spalding) and Skegness Community Hospital.
* Complement the role of Clinical Team Leads on the ward, undertaking appropriate clinical activity as required.
* Be responsible for performance and target levels for the Community Hospitals, as identified by LCHS.
* Act as Complex Case Manager for a designated caseload, ensuring all patients access and progress through their individual care plans.
* Provide complex therapy advice, assessment and treatment to patients under case management.
* Develop and maintain specialist areas of expertise and an extended scope of practice.
* Create training packages and documentation to support the delivery of high‑quality care.
* Independently assess patients with complex specialist needs to determine overall service provision and onward referrals.
* Maintain high standards of care through effective communication with patients, carers, GPs and other health professionals.
