Overview
The Wellbeing Centre at Edenbridge Memorial Health Centre supports adults with long‑term conditions, dementia and age‑related needs to stay well, independent and safe at home. Working Monday to Friday within a warm, multidisciplinary team aligned to local Primary Care Networks, you’ll play an important part in delivering proactive, person‑centred care to people who benefit from regular monitoring and support.
We welcome applicants with or without previous healthcare experience. If you’re new to care, you can join us in a Band 2 developmental role, with a clear pathway to progress to Band 3 once you’ve achieved the required competencies. You’ll be fully supported with training, supervision and hands‑on learning to help you grow confidently into the role.

Responsibilities
* As a Band 3 Healthcare Assistant—or a Band 2 working towards Band 3—you’ll help deliver personalised wellbeing interventions that follow best‑practice principles. Your work will focus on prevention, early identification of deterioration and empowering patients to manage their own health more confidently.
* Monthly observations and routine health checks
* Basic education and information to help patients stay safe and independent
* Health promotion and support for long‑term condition management
* Regular monitoring and onward referrals when needed
* You’ll work closely with nurses, therapists, GPs and wider community teams to provide joined‑up care for adults over 18. The team culture is collaborative, encouraging and centred on helping patients maintain their wellbeing and avoid unnecessary hospital admissions.
