Role overview
The Home Assessment Team delivers on the national priority agenda of Discharge Without Delay through integrated working between local authority, health and third sector partners. As an Occupational Therapist you will support the people of Clydesdale to live at home independently, using clinical and rehabilitation skills, person‑centred planning and by leading effect‑based improvements in service delivery.
Key responsibilities
* Implement occupational therapy principles within community practice, maintaining high standards of professional practice and provision.
* Conduct comprehensive specialist assessments in the community setting in partnership with clients and/or their advocate.
* Formulate and coordinate care plans for complex disability‑related issues in line with community care procedures.
* Provide direct occupational therapy interventions to address assessed needs.
* Conduct specialist functional or environmental assessments that complement departmental goals.
Qualifications
* Diploma from the College of Occupational Therapists, or an equivalent BSc or higher degree.
* Current registration with the Health Professions Council.
* Experience in community‑based occupational therapy is desirable.
