 
        
        We are looking for a part time clinical pharmacist to join our existing team, ideally for a minimum of 22.5 hours per week
The primary purpose is to support a Primary Care Network (PCN) and its member practices in the Pudsey / Bramley area in all aspects of medicines optimisation. The successful recruit will likely work both within the PCN and also within a practice for part of their working week
The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines optimisation across the PCN. This will include structured medication reviews with direct patient contact and may include contributing to:
Management of long-term conditions
Reviewing systems for safer prescribing
Action acute prescription requests
Addressing both the public health and social care needs of patients
Contributing to achievement of QOF and locally commissioned quality improvement schemes.
Undertaking clinical audit
Developing, promoting and implementing the high quality, evidence based and cost-effective use of medicines in primary care in a way that maximises the benefits of medicines to patients and service users and which minimises risks (clinical, legal and financial).
This list is not exhaustive and additional duties may be decided by the PCN Leadership Team and the Lead Clinical Pharmacist.

We also have our own premises (non patient facing) where most of the staff are based day to day (if working remotely) which lends itself beautifully to instant access, in house MDT working.  * Prior completion of CPPE pathway
