Job Summary
As part of the Rotherham Healthwave team, our Health and Wellbeing Coaches support people from the local community to improve how they manage their physical and mental health. By working with people one-to-one or in a group setting, you will help people develop the knowledge, skills, and confidence to change their health-related behaviours, helping to improve their overall quality of life, health, and wellbeing. These behaviours can include dietary advice, smoking cessation, and physical activity. You will also be responsible for maintaining up-to-date and accurate clinical records. The role requires coaching skills, patience, and empathy to support people in reaching their goals. A person-centred approach is critical to the service's success, and although the role can sometimes be challenging, it is also very rewarding.

About Us
Connnnect Healthcare Rotherham Community Interest Company (CIC) is a federation of 28 GP practices in Rotherham. The organisation is led by a Board of representatives from general practice in the Borough, with executive leadership from a Medical Director on a part-time basis. The organisation currently delivers extended access primary care services in Rotherham and a Physio First service in partnership with the local acute and community trust, the largest of its kind currently operating. In addition, the organisation is supporting the development of primary care networks and manages delivery of training and development to large cohorts of the general practice workforce.

Job Description
Job responsibilities:
* Manage and maintain a caseload of clients throughout their chosen lifestyle intervention.
* Develop individual, evidence-based, tailored support plans based on assessed needs, including individual and group-based interventions, signposting, and referrals to other health, community, and voluntary services.
* Work with clients to set SMART goals using evidence-based knowledge, NICE, and NHS guidelines.
* Take weights and waist measurements in a compassionate and confidential manner during nutrition/weight management sessions.
* Contribute to monitoring and evaluation, including completing the required data set and following up with clients who do not engage, and identifying high-risk cohorts.
* Ensure patient-related documentation is entered into electronic health records with high accuracy and completeness.
* Maintain data quality and assist in improving data collection with managers and IT support.
* Assist managers and clinicians in achieving contracted targets by ensuring data is entered accurately and promptly.
* Manage time and priorities when providing service; undertake other reasonable tasks as directed.
* Maintain confidentiality in line with the organisation's policy; support safe and effective team practices; work flexibly with the team; attend community events as required.
* Raise barriers to safe and effective working with the line manager; attend mandatory training and comply with health and safety regulations.
* Design/upload and staff rotas for the Weight Management and Nutrition team.
* Work as part of the wider team to deliver the Rotherham Healthwave service.
Person Specification
Essential:
* GCSE (or equivalent) grade C or above in English and Maths.
* Working knowledge of Microsoft Office.
* Experience in managing highly confidential and sensitive information.
* Caring and compassionate with empathy for service users.
* Ability to work as part of a team; work under limited supervision; access support and escalate issues as necessary.
* Ability to travel between sites with own transport and hold a full UK licence.
Desirable
* Understanding of common mental health problems.
* Knowledge of behaviour change models and motivational interviewing.
* Experience providing help or counselling one-to-one or in groups.
* Experience with electronic health records systems, particularly SystmOne.
* Qualification in health and wellbeing or health coaching.
* Knowledge of health promotion and health inequalities.
Qualifications
Essential: GCSE English and Maths at grade C or above.
