Responsibilities include:
1. Responding promptly to minimize patient time in the Emergency Department (“ED”) and reduce overcrowding.
2. Taking behavioral health histories and collecting information on current functioning and contributing factors.
3. Identifying current providers, state agency involvement, and health-related social needs.
4. Providing support and updates to patients and families throughout the process.
5. Collaborating with the psychiatric team on admissions, authorizations, and coordination of care.
6. Arranging inpatient admissions, transfers, transportation, and updating crisis or safety plans.
7. Sharing information with providers and supporting discharge planning and community linkage.
8. Completing documentation and maintaining community resource lists.
9. Participating in process improvement and delivering service excellence.
Qualifications include a Bachelor’s degree in human services (Master’s preferred), 1-3 years of relevant experience, proficiency with electronic health records and Microsoft Office, excellent organizational skills, Crisis Prevention Intervention training within 1 month, and current BLS certification.
Position details: 32 hours/week, rotating evenings, weekends, and holidays, located in Falmouth, MA, at Falmouth Hospital.
