Key Responsibilities
Project Set up: To spearhead the launch of a new virtual pulmonary hypertension service, including operational planning and optimising patient care.
Trust Engagement: Coordinate with NHS Trusts and stakeholders to ensure effective implementation of the patient support programme.
Patient Training and Support:
* Educate and initiate PH patients on therapy.
* Provide ongoing virtual support, training and education to PH patients and their caregivers.
* Working as an advocate for your patient’s needs, you’ll have the autonomy to manage your own diary, ensuring time to build trusting relationships with your patients, carers and other stakeholders.
Clinical Excellence: Maintain exceptional standards of care, documentation and compliance with clinical governance.
About You
* You’ll be a current registered nurse with proven experience in pulmonary hypertension nursing. (Ideally minimum Band 7).
* Proven ability to work autonomously and deliver care on a virtual basis.
* Comfortable with digital platforms and remote patient engagement.
* Excellent communication and relationship building skills.
* Extensive understanding of the NHS is a must, and previous experience of successful nursing within the pharmaceutical healthcare industry will be a distinct advantage.
