What You Will Do
* Provide high-quality social work support to adults in the community, with a focus on older persons.
* Carry out comprehensive assessments and develop person-centred care plans.
* Manage and review caseloads to ensure safe and effective interventions.
* Work collaboratively with families, carers and partner agencies.
* Maintain accurate, timely and compliant case records.
* Use Liquid Logic to record assessments, plans and outcomes effectively.
Requirements
* Qualified Social Worker with registration to the relevant professional body.
* Minimum two years’ post-qualification experience in adult social care.
* Experience in community-based adult services.
* Working knowledge of Liquid Logic preferred.
* Strong assessment and care planning skills.
