An exciting opportunity has arisen for a dynamic and experienced Advanced Community Nurse Practitioner to join the Enfield District Nursing service.
The Advanced Community Nurse Practitioner is responsible for managing healthcare within a specific geographical area in line with the Primary Care Network (PCN). They provide clinical leadership to ensure high-quality and standard care service delivery.
The post holder aims to improve care pathways for patients, carers, and families, working in conjunction with statutory and voluntary services.
This role requires a senior nurse with a Specialist Practitioner District Nursing Qualification or equivalent, with significant community nursing experience to offer expert clinical advice to the District Nursing Team.
Key responsibilities include:
1. Providing clinical leadership for all patients on the district nursing caseload through autonomous practice, including risk management.
2. Collaborating with professionals, carers, and relatives to understand patients’ physical, emotional, and social needs.
3. Conducting physical examinations, history taking, diagnosis, prescribing, and care planning.
4. Developing and maintaining personalized care plans based on comprehensive assessments.
5. Ordering and interpreting investigations and tests, supporting clinical decision-making.
6. Maintaining prescribing competence and engaging in ongoing professional development.
The role is based at North Middlesex University Hospital NHS Trust, serving a diverse population with various specialist services, including HIV, cardiology, cancer treatment, and a dedicated 0-19 service for children and young people.
Applicants must be Registered Nurses with community experience managing complex needs and advanced assessment skills. They should have completed the DNSP qualification and be an independent prescriber or working towards it.
Additional responsibilities include comprehensive history taking, physical examinations, diagnosis, treatment planning, collaboration with multidisciplinary teams, empowering patients and families, and ensuring quality through audits and reviews.
