Location: Town Hall, St Ives Rd, Maidenhead, Berkshire, SL6 1RF
Pay Rate: Up to £38 per hour
Working Pattern: Hybrid (Minimum 2-3 days per week in the office)
Overview
The Royal Borough of Windsor & Maidenhead is looking for an experienced Occupational Therapist to join their Hospital Team. The role involves delivering an outcome-focused assessment service to adults with physical disabilities or age-related needs, supporting safe and timely hospital discharges.
Responsibilities
* Carry out outcome-focused assessments for adults with physical disabilities or age-related conditions.
* Support hospital discharge planning in collaboration with health and social care professionals.
* Work closely with multidisciplinary teams to ensure effective care planning.
* Maintain accurate case records and documentation.
Requirements
* Qualified Occupational Therapist with relevant professional registration.
* Local Authority and hospital discharge experience required.
* Knowledge of the Mosaic case management system is desirable.
* Must be a driver with a valid driving licence and access to a vehicle for work purposes.
* Ability to work hybrid, with 2-3 days per week in the office.
