The unit provides a Comprehensive Geriatric Assessment to the frail elderly patients living within the Buckinghamshire area, additionally we have an Intravenous Infusion Lounge.
The post involves working within a Multidisciplinary Team to provide nursing interventions to frail elderly patients to avoid hospital admission, as well as, facilitating discharges from the acute settings.
You must be innovative in your approach and willing to work with other members of the Multidisciplinary team and to ensure a high-quality service delivery. You must be willing to develop your own knowledge and skills, you will be supported in your personal development with training.
Experienced and newly qualified staff nurses will be considered for the post. Working days are Monday-Friday, no weekends, nights, or bank holidays
Main duties of the job
Acts always in accordance with the NMC Code of Conduct, and within the principles of the scope of Professional Practice and ensures other members of the team also work within these.
Maintain professional working relationships with all members of the Multidisciplinary Team and effectively contribute to the team’s continual development.
Undertakes the nursing assessment of the patient including risk assessments e.g. Moving and Handling and nutrition
Undertake drug administration and medicine management.
Ensures planned nursing care is delivered safely and effectively taking into account the views of the patient and their relatives. Advise on health promotion and the prevention of illness.
Maintains absolute confidentiality in respect of all medical and nursing records and interpersonal issues with colleagues.
Interpret the significance of the patient’s clinical observations and records and taking appropriate action but ensuring senior nursing staff are kept always informed.

2. EDUCATION, QUALIFICATIONS & TRAINING
Essential criteria
1. Standards for student supervision and assessment (SSSA)
2. Ability to work as part of a team and on own initiative
3. Registered Nurse/Awaiting NMC PIN
Desirable criteria
4. Cannulation and Venepuncture
5. Previous experience as a band 5 in acute area
6. Evidence of further continued professional development
7. Post qualification experience in relevant speciality
8. Hold / be working towards an NMC approved mentoring qualification
