Why not try this exciting new opportunity to work on the Multi-Disciplinary Assessment Service at Wycombe hospital? The unit provides a Comprehensive Geriatric Assessment to the frail elderly patients living within the Buckinghamshire area, additionally we have an Intravenous Infusion Lounge.

The post involves working within a Multidisciplinary Team to provide nursing interventions to frail elderly patients to avoid hospital admission, as well as, facilitating discharges from the acute settings.

You must be innovative in your approach and willing to work with other members of the Multidisciplinary team and to ensure a high-quality service delivery. You must be willing to develop your own knowledge and skills, you will be supported in your personal development with training.

Experienced and newly qualified staff nurses will be considered for the post. Working days are Monday-Friday, no weekends, nights, or bank holidays.

Main duties of the job

Acts always in accordance with the NMC Code of Conduct, and within the principles of the scope of Professional Practice and ensures other members of the team also work within these.

Maintain professional working relationships with all members of the Multidisciplinary Team and effectively contribute to the team’s continual development.
Undertakes the nursing assessment of the patient including risk assessments e.g. Moving and Handling and nutrition

Undertake drug administration and medicine management.

Ensures planned nursing care is delivered safely and effectively considering the views of the patient and their relatives. Advise on health promotion and the prevention of illness.

Maintains absolute confidentiality in respect of all medical and nursing records and interpersonal issues with colleagues.
Interpret the significance of the patient’s clinical observations and records and taking appropriate action but ensuring senior nursing staff are kept always informed.

Education, Qualification & Training

Essential criteria

10. Registered Nurse/Awaiting NMC PIN
11. Evidence of further continued professional development
12. Post qualification experience in relevant speciality
13. Hold / be working towards an NMC approved mentoring qualification

Experience

Essential criteria

14. Ability to understand the role of a registered nurse
15. Ability to work as part of a team and on own initiative
16. Awareness of current professional nursing issues
17. Demonstrates understanding of the importance of audit/quality

Desirable criteria

18. Post registration experience working in an acute hospital setting

Skills, Abilities & Knowledge

19. Good verbal and written communication skills and ability to negotiate
20. Ability to manage and prioritise own caseload with supervision
21. Ability to solve problems and use initiative
22. Ability to develop clinical skills
23. Willingness to undergo training (e.g. medications)
24. Basic IT skills
