An exciting opportunity has arisen to recruit a Band 4 Care Coordinator to join the busy Prostate Clinical Nurse Specialist team at the Royal Surrey County Hospital. The post holder will support the delivery of the Prostate Cancer Supported Self‑Management (SSM) pathway, working closely with the Prostate CNS team to ensure safe, effective, and patient‑centered care.
Responsibilities
* Act as the key point of contact for patients on remote monitoring pathways.
* Coordinate PSA tracking and safety‑netting processes.
* Complete holistic assessments and implement personalised care plans.
* Signpost patients, carers and families to appropriate support agencies.
* Assist the CNS team by undertaking delegated clinical and administrative tasks.
* Monitor patients on an active surveillance pathway, including regular blood tests and MRI scans.
* Provide emotional and practical support to patients diagnosed with prostate cancer.
* Collaborate with the multidisciplinary team and work autonomously within occupational guidelines.
Qualifications
* Excellent communication skills with the ability to manage a large patient group dealing with sensitive information.
* Previous oncology experience is advantageous.
* Ability to work independently and as part of a multidisciplinary team.
* Initiative, organisational skills and a compassionate approach.
