What’s the Role About?
In this unique role, you’ll track patient pathways from the referral through to their first stage of treatment, analysing large amounts of data each day to ensure every step is accurately reported onto our Patient Tracking Lists. Your attention to detail will ensure that all specialties we monitor have the critical information they need to meet national standards. It’s a fast‑paced, rewarding role where your work will have an impact on patient satisfaction and care.

Key Duties
* Perform daily validation of the 18 Weeks/RTT PTL (Patient Tracking List) database to ensure accuracy and compliance with RTT standards.
* Complete detailed validation & error reports, and identify and rectify errors as necessary.
* Validate patient letters and documents across multiple specialties, ensuring all information is accurate and up‑to‑date.
* Provide an accurate monthly representation of RTT standards to the Trust.
* Communicate confidently and professionally with internal and external staff members, ensuring smooth communication across teams.
* Carry out personal workstreams, ensuring all agreed timescales and goals are met.
* Participate actively in team meetings and contribute ideas, fostering a positive and open team culture in alignment with the Trust’s values.
* Assist with improvement and development in the timely capture of accurate data across the Trust.
* Update QCPR and identify and raise risks to compliance.
* Validate 18‑week breaches for admitted and non‑admitted patients and update QCPR accordingly.
* Extract pathway information from clinical correspondence in shared electronic folders and on ALMA, Evolve, Bluespiers or in paper notes.
* Use appropriate computer packages including Bi Portal, Prism, GI Tool to extract data for analytical purposes.
* Escalate inaccuracies, inconsistencies and gaps in RTT data collection.
* Take appropriate actions for validation/exception reports, ensuring pathway information is obtained, corrected and issues escalated as required.
* Ensure all information relating to the patient RTT pathway is communicated effectively by the most appropriate medium.
* Promote the importance of patients’ right to access services within maximum wait times and support understanding of current 18‑week RTT measures and standards.
* Escalate to senior staff all risks of potential breaches of RTT pathways identified as a result of analysis.
* Assist in the collection of data for audit purposes and support management teams to identify areas that require pathway improvements.
* Assist in ensuring that the working practices within the team effectively support achievement of the 18‑week RTT measures and standards.
* Fulfil above functions by efficient use of computerized systems, email and manual systems.
* Audit the effectiveness of procedural documentation to ensure practice matches procedure.
* Continuously evaluate the effectiveness of own work streams and their agreed timescales.
* Participate in planned rotation between sites as necessary.
* Take an active role in team meetings as required.
